The Massachusetts Department of Public Health (MDPH) is applying to the Health Resources & Services Administration (HRSA) for the funding opportunity: Transition for Youth with Autism and/or Epilepsy Demonstration Projects, funding number: HRSA-24-042. The purpose of this grant is to improve outcomes for transition from child to adult serving systems for youth with autism and/or epilepsy who have complex health and social needs and require a higher level of family support and coordination (YAES). The grant will fund demonstration projects that require awardees to implement and evaluate a strategy to improve transition outcomes for YAES in an innovative, sustainable, and scalable way. The purpose of this notice is to publicize the MDPH's intent to contract with the Boston Children's Hospital BRIDGES Program pending final approval with the federal funding agency.
